Dedicated Sleep and Implants Partnership

We are excited to announce our partnership with a medical specialty group, Dedicated Sleep and Implants, allowing us to offer in-network benefits for some BCBS PPO medical insurance plans for medically necessary Full Jaw reconstruction. This means that you can now access our services with the financial ease of your insurance benefits.

 

How the BCBS Authorization Process Works

To ensure you understand each step, we have outlined the BCBS authorization process below:

Step 1: Benefit Verification

First, we will obtain a benefit verification form (BV) from BCBS to determine if your plan is eligible. We will need a picture or copy of the front and back of your BCBS medical insurance card and your driver’s license.

Step 2: Medical Exam Appointment

Once we confirm your plan, we will schedule a Medical Exam appointment with us. You will receive an email with a link to complete the necessary questionnaires before your appointment. This form must be filled out before your appointment.

Step 3: In-Office Visit

During your in-office visit, you will undergo a medical exam which includes an interview, simple measurements, and some dental x-rays. This process will take about an hour.

Step 4: Pre-Authorization

Using the information and records we have gathered, we will submit a pre-authorization to your medical insurance. We need to establish medical necessity for the procedure, and this process usually occurs after authorization from your medical insurance has been received. You may also need to schedule a telemedicine call with one of our medical directors. As part of being a specialty group, this information reflects Dedicated Sleep and Implants, not WestLake Dental Care.

Step 5: Staged Procedures

Once approved, we will contact you to schedule your staged procedures and review your financials. As part of being a specialty group, this information reflects Dedicated Sleep and Implants, not Westlake Dental Care.

Important Information

Please note that this type of surgery is billed in phases, as your treatment is provided. Any changes to your medical insurance plan or additional services not paid by BCBS may result in additional out-of-pocket costs. We recommend informing your employer to make sure any changes do not affect your coverage.
